Code 99453 – Setup for RCM

Initial device setup and patient education.
Key Billing Requirement: One-time per patient, per episode of care.
Essential Documentation: Documented patient consent, device identification number, date of device delivery, and date(s) of patient education on use.

99454 – Device Supply
+
Device supply and daily data transmission/monitoring.
Key Billing Requirement: Data must be transmitted on at least 16 days within a 30-day period.
Essential Documentation: Log of daily readings confirming the 16-day requirement was met. The device must meet the FDA definition of a medical device.

99457 – 20 Minutes of Monitoring Time

First 20 minutes of remote physiologic monitoring treatment management services.
Key Billing Requirement: A minimum of 20 minutes of clinical staff time per calendar month. Requires at least one live, interactive communications.
Essential Documentation: Detailed time logs (start and stop times or total duration), name and credentials of staff, and a summary of the interactive communication and care management activities performed.

99458 – Additional Monitoring Time

Each additional 20 minutes of RPM treatment management services.
Key Billing Requirement: Billed in 20-minute increments after the initial 20 minutes for CPT 99457 have been met.
Essential Documentation: Same as CPT 99457. Time must be tracked separately for each increment and cannot be rounded up. Documentation must clearly support the additional time spent.
